Akron Children’s Hospital will be offering TDaP & Meningitis vaccination clinics at Coventry Schools on Tuesday March 19, 2024 for students in 10th and 6th grade. These vaccinations are REQUIRED for school enrollment by the State of Ohio.
If you decide to have your child vaccinated in the upcoming clinic please complete the Vaccine Consent Form. Please return the CONSENT FORM for High School Students or CONSENT FORM for Middle School Students to the school no later than Monday, March 11th to secure your child’s vaccination.
