TDaP & Meningitis Immunization Clinic
With the new immunization requirements for all 7th and 12th graders - Coventry Schools has arranged for Health Heroes' TDaP & Meningitis Immunization Clinic to be held at the Middle School August 18th from 5 pm - 7pm and August 19th from 9 am- 11 am.
Note: All forms must be COMPLETELY filled out and signed before arriving to the clinic. Also, for students who are required to have both the Meningitis and TDaP- a separate consent form must be completed for each immunization.
